Frontline Health Portal

The ICSD has licensed the Frontline Health Portal to allow families to conduct their children's daily health screenings on a secure, remote platform. On this page, you will find instructions for activating your account and completing the health screening, as well as some helpful troubleshooting tips.

Health Portal Instructions

Parents/guardians can activate their accounts and complete the daily health screening via Frontline's Mobile or Desktop application.
  • Mobile: Go to the Apple App Store or Android Apps on Google Play, search for Frontline Health Portal, and download the app.
  • Desktop: Go to the ICSD Health Portal login page to access the activation portal.
Activate Your Account

After you have downloaded the mobile app or used the secure link to access the desktop app, follow the steps below to activate and access the Frontline Health Portal (download a PDF of the activation instructions or watch a step-by-step demonstration).

  1. Enter your email address and select the blue button, “Send me a temporary password.”

    Note: You will need to activate your account as a family with your personal email address that the district has on file for you in SchoolTool. Instructions for verifying and requesting changes to your contact information in SchoolTool can be found on our Parent/Caregiver Communications page.

  2. Open the email in your inbox, copy the temporary password, and click on the link to go back to the Health Portal login page. (Please check your email’s spam folder if you have not received the email in 10-15 minutes.)

  3. Enter your username (email address) and the temporary password, then follow the steps to update your password and continue to the Portal Terms of Use.

  4. Click the checkbox beside “I have read and agree to the Terms of Use and Electronic Service Agreement” and follow the eSignature instructions (be sure to leave out any spaces between /S/ and your name), then select the “Confirm my agreement” button.

  5. You will be prompted to assign student(s) to your account. Provide your child's last name, student ID, and date of birth, then click “Add this student.”

    Note: Your child's student ID is a 9-digit number beginning with 71000, which can be found next to their contact information in SchoolTool. For assistance, contact your child's school or email ICSD Communications.

  6. Once you've added each of your children, select “I’m done adding students” to complete your account activation.

Complete the Daily Health Screening
Desktop Users
  1. Go to the ICSD Health Portal login page
  2. Click the form icon on the row for the Daily COVID-19 Health Check to open the questionnaire
  3. Check any prompts that are true for your child (see screenshot below)
  4. Select "Yes" from the dropdown menu to complete the attestation
  5. Click "Submit"

Mobile App Users
  1. Log in to the Frontline Health Portal app on your mobile device
  2. Tap the menu icon in the upper left to display side navigation
  3. Select "My Students"
  4. Tap the name of the child whose questionnaire you will be completing
  5. Return to the side navigation and tap "To Do/History"
  6. Select "Daily COVID-19 Health Check"
  7. Check any prompts that are true for your child
  8. Select "Yes" from the form field to complete the attestation (see screenshot below)
  9. Tap "Done," then "Submit"

Troubleshooting Tips for Families

Issue: "We were unable to send an email to the address you provided.”
  • Email the district with your first and last name, child/ren’s first and last name(s), and the correct email that should be on file
Issue: Temporary password email has not shown up after 15 minutes.
  • Inform the district that you have not received your temporary password email
  • Use the temporary password that the district gives you to log in to the Portal
  • Your username is the personal email that was proved by the school district and that you used to activate the portal account
Issue: “We were unable to add this student to your Health Portal account.”
  • Confirm you have spelled the student’s last name correctly, have the correct student ID, and the correct date of birth
